Ace Care Team Services recruits and places three core categories of healthcare professionals. Below is
a detailed profile of each role, their responsibilities, and what you can expect from every professional we
place.

HHA - Home Health Aide

Home Health Aides are trained healthcare paraprofessionals who provide personal care,
supportive services, and health monitoring to clients in their homes or community settings.
HHAs are the frontline of in-home healthcare, offering both clinical support and meaningful
companionship.

Assisting clients with all activities of daily living (ADLs) including bathing, dressing, grooming,
and toileting.
Administering medication reminders and monitoring for medication compliance.
Taking and recording vital signs such as blood pressure, pulse, temperature, and respiration.
Assisting with prescribed exercises, physical therapy routines, and mobility support.
Preparing nutritious meals in accordance with dietary restrictions and care plans.
Performing light housekeeping duties to maintain a clean and safe home environment.
Providing emotional support and companionship to reduce social isolation.
Observing and reporting changes in client condition to supervising nurses and care coordinators.
Assisting with transportation to medical appointments (in coordination with NEMT services).
Documenting care activities accurately in accordance with agency and regulatory standards.

Valid Home Health Aide (HHA) certification in the state of employment.
Completion of a state-approved HHA training program (minimum required hours per state).
Current CPR and First Aid certification.
Clean criminal background and satisfactory reference history.
Strong communication skills and demonstrated compassion for patient care.
Ability to follow written and verbal care plan instructions.
Reliable transportation and flexible scheduling availability

CNA - Certified Nursing Assistant

Certified Nursing Assistants are state-certified allied health professionals who provide direct
patient care under the supervision of licensed nurses. CNAs are a critical component of
quality care in both clinical and home settings, performing clinical tasks that Home Health
Aides are not authorized to perform.

Providing direct patient care including bathing, oral hygiene, skin care, and positioning
Monitoring and accurately recording vital signs and patient intake/output
Assisting licensed nurses with clinical procedures and patient assessments
Safely transferring and repositioning patients to prevent pressure injuries and promote comfort
Operating and maintaining basic patient care equipment (hoyer lifts, transfer belts, etc.)
Collecting and labeling patient specimens for laboratory analysis
Assisting patients with ambulation, range-of-motion exercises, and rehabilitation activities
Reporting changes in patient status promptly and accurately to supervising nursing staff
Maintaining patient dignity, privacy, and comfort at all times
Completing nursing assistant documentation and care notes accurately and timely
Providing emotional support, encouragement, and social engagement to patients and residents

Active, unencumbered Certified Nursing Assistant (CNA) certification on the state registry
Completion of an approved state CNA training and competency evaluation program
Current BLS/CPR certification from an accredited provider
Clean criminal background check and state nurse aide registry clearance
Minimum 6–12 months of clinical or post-acute care experience preferred
Strong clinical observational skills and accurate documentation abilities
Ability to work under the direction of RNs and LPNs in fast-paced environments
Demonstrated professionalism, reliability, and commitment to patient-centered care

PCA - Patient Care Assistant

Patient Care Assistants provide essential supportive care services to individuals with
disabilities, chronic illness, or age-related needs in home and community-based settings.
PCAs work closely with clients and their care teams to promote independence and quality of
life.

Assisting clients with personal hygiene, grooming, bathing, dressing, and toileting
Supporting clients with meal preparation, feeding assistance, and dietary monitoring
Providing mobility assistance including transfers, positioning, and ambulation support
Accompanying clients to medical appointments, community events, and recreational activities
Performing light housekeeping, laundry, and general home management tasks
Assisting with medication reminders as directed by the care plan
Monitoring and reporting client health status changes to supervisory staff
Providing companionship and emotional support to promote mental and social well-being
Facilitating communication for clients with limited verbal ability (AAC, assistive devices)
Supporting clients in achieving personal goals related to independence and community
participation

High school diploma or GED equivalent required
State-issued PCA certification or completion of an approved training program
CPR and First Aid certification (current)
Clean criminal background check and satisfactory reference verification
Prior experience in personal care, home care, or a related healthcare setting preferred
Physical ability to assist with lifting, transferring, and supporting clients as needed
Patience, empathy, and a genuine commitment to person-centered, dignified care
Strong organizational skills and the ability to follow individualized care plans
