Key Objectives

Assist patients with daily living activities to promote comfort and dignity.
Support healthcare plans by monitoring patient conditions and reporting changes.
Foster a safe, clean, and supportive home environment for patients.
Build trusting relationships with patients and their families through compassionate care.

Responsibilities

Provide personal care services such as bathing, dressing, grooming, and toileting assistance.
Help patients with mobility, including transferring and positioning.
Assist with medication reminders and basic health monitoring (e.g., temperature, pulse).
Prepare meals and assist with feeding as needed, ensuring dietary requirements are met.
Perform light housekeeping tasks to maintain a clean and safe living environment.
Accompany patients to medical appointments or errands when required.
Document care provided and communicate effectively with nursing staff and family members.
Adhere to all safety, confidentiality, and infection control protocols.

Qualifications & Requirements

High school diploma or equivalent preferred.
Previous experience as a Home Health Aide or in a related caregiving role is an advantage.
Certification as a Home Health Aide (HHA) or willingness to obtain certification as required by state regulations.
Basic knowledge of healthcare practices and ability to follow care plans.
Strong interpersonal and communication skills with a patient, empathetic approach.
Ability to work independently and manage time effectively.
Physical ability to assist with mobility and perform household tasks.
Reliable transportation and flexible availability to meet patient needs.
